The undersigned states as follows:
I am over the age of 18 years and otherwise legally competent. I certify under penalty of perjury under the laws of the State of Delaware that the foregoing is true and correct.
Authorization is hereby granted to Sound Equity Inc and its affiliates to obtain a credit report through a credit reporting agency chosen by Sound Equity Inc ("SEI")
for each of the entities and individuals completing this form. Completion of this form authorizes SEI to release to the credit reporting agency a copy of
my credit application, and authorizes SEI or any affiliate or credit reporting agency to obtain a copy of my credit report and any information regarding my
employment, savings accounts, and outstanding credit accounts (mortgages, auto loans, personal loans, charge cards, credit unions, etc.). Authorization is further granted to SEI
or any affiliate or credit reporting agency to use a reproduction of this authorization, if necessary, to obtain any information regarding the above mentioned
information and any such reproduction of this credit report authorization and release made by reliable means (for example, photocopy or facsimile) is considered an original. Upon
request, SEI will provide applicant with the name and address of any credit reporting agency used to prepare report.
